How to use the template
- Record the date and local time after an actual administration.
- Use the exact medication name and formulation so different products are distinguishable.
- Write the administered amount together with its unit. Keep a separate row for each administration.
- For an injection, record the site you used. For oral medicine, leave the site blank.
- Review the history before an appointment and share only what you choose.
What a useful note looks like
Illustrative example: “Recorded after taking it; used the left thigh; question for my next appointment about traveling with my medication.” This is a fictional note, not a dosing instruction or a customer's result.
CSV or app: which fits your routine?
A spreadsheet is useful when you want an editable table and a manual record. It will not send reminders or automatically read Apple Health. Capydose adds dose reminders, history, symptoms, weight and app reports. Neither this template nor a downloaded CSV can be imported into Capydose.
What if I missed a dose?
Do not add a missed dose as if you took it. Record the situation in your notes and follow your medication's current instructions or ask your healthcare professional. This diary cannot decide when to take the next dose.
