Make your next medicines review easier to prepare

Whether your prescription comes through NHS care or a private service, a personal diary can help you explain what you recorded between appointments. Bring the exact product and dispensing instructions, your own dates and the questions you have not yet answered. The diary does not determine entitlement, eligibility or a prescribing plan.

The review checklist below focuses on preparing that conversation. A separate Yellow Card worksheet helps organise observations if you choose to report a suspected side effect. Reporting and getting medical attention are different tasks; a diary or submitted report does not replace care.

  • Keep a list of prescribed medicines and the questions you want to discuss.
  • Write the unit with each weight entry; retain the original measurement if converting it.
  • Use dates and your own descriptions rather than assuming what caused a symptom.
