Build a one-page overview
- Exact medication name, route and the instructions you were given.
- Administration dates, including any date you are unsure about.
- The observations you most want to discuss, with onset and duration when known.
- Questions about the plan, the written instructions or what to track next.
Make missing information visible
A blank field is different from “no symptoms.” If you did not record a day, leave it missing. If you entered a note from memory, say so. Accurate limits make your record more interpretable than a report that appears complete but contains guesses.
