Discussion sheet
Prepare for your GLP-1 appointment
Put your most important question first, add observations with dates, and choose which records to bring. You decide what goes into the summary.
Build a discussion sheet
Names and contact details are optional. Focus on the questions and records that will make the conversation useful.
Your entries stay in this page. These tools do not upload entries or save a draft. Download or print what you need before leaving. You decide where an exported file is saved. Website privacy.
My appointment discussion sheet
This sheet contains my own notes for discussion. It is not a treatment recommendation.
Printable appointment checklist
Use these prompts if you prefer to write by hand.
- My main question:
- What I noticed, and on which dates:
- Current medicines and supplements:
- Other questions:
- Records to bring: dose history, symptom diary, weight records, and any reports requested by my clinician.
- During the appointment: write down the agreed next steps and whom to contact with follow-up questions.
Examples of questions to adapt
These are general discussion prompts, not personalized medical advice:
- “Here are my symptoms and the dates. Which details would you like me to track?”
- “Where can I find the instructions for my exact medication and formulation?”
- “What should I do if I have a question between appointments?”
- “Which records should I bring next time?”
Using your Capydose report
In Capydose, open the report/export area, select a date range and the sections you want to include, and preview the report before saving or sharing it. PDF is useful for a readable appointment summary; CSV provides rows for a spreadsheet. Sharing a report is your choice and does not automatically grant a clinician access to your app.
This page prepares your notes. It does not contact a clinic, book an appointment, recommend a dose or replace a consultation.
