Prepare GLP-1 symptom notes for a PvPI report
Organise the facts you know, find the official patient form and understand what remains separate from saving a note in Capydose.
Care first; documentation and reporting are separate
A symptom noted after a medicine is an observation, not proof that the medicine caused it. If you are concerned or need urgent help, contact an appropriate healthcare professional or emergency service. Do not wait for a complete diary, a response from a reporting programme or your next routine appointment to seek necessary care.
The Indian Pharmacopoeia Commission provides patient-reporting resources through PvPI, including a Hindi consumer form and information on reporting assistance. You can use those official resources to find the current route. Saving or exporting a Capydose note does not submit a report to PvPI, CDSCO or your doctor.
Build a chronology from facts you can explain
- Name the medicine. Use the pack or prescription for the exact name and presentation. If you know the batch and expiry, keep those alongside the original information.
- Describe the event. Write what you noticed, when it began, how long it lasted and whether it was still happening at the time of the note. Avoid replacing your description with a diagnosis copied from a search result.
- Show the time sequence. Separate an administration date, symptom onset and the date you wrote the entry. Note when a date is approximate.
- Record actual action. Include a call, appointment or advice received with its date. Do not add a treatment change unless it actually happened, and do not use the worksheet to choose a change.

Use the file for private preparation or print the table from your browser. Its columns do not reproduce every field in the official reporting form. Keep dates as YYYY-MM-DD in this worksheet, then follow the date format requested by the official form when transferring information. Leave unknown details marked unknown; a blank value is different from “no symptom”.
Move from notes to the official reporting route
Open the IPC reporting-resources page and use the patient form or reporting-assistance route it identifies. The linked Hindi consumer form also includes English labels. Read both pages and the current instructions before sending information. Check the destination directly on the official website rather than relying on an old forwarded message.
The official consumer form says reports can be made even when not all information is available. Our worksheet is optional preparation, not an additional requirement. Share the personal and contact details requested by the official service through that service, rather than posting your draft publicly. Keep any acknowledgement or reference you receive, and note if you later provide a correction.
Fictional writing example: “Discomfort noticed in the evening; start time uncertain. Wrote this note the next morning and contacted the clinic.” Use it only as an example of clear writing, not as a patient story or a diagnosis. Recording uncertainty is more useful than filling in a precise time you do not remember.
Keep your personal record in Capydose
Capydose helps organise dose and symptom history and produces PDF or CSV reports you choose to share. It does not diagnose an adverse reaction or transmit a PvPI report. The app is available for iPhone with iOS 18 or later; tracking needs Premium after onboarding. Android is in review. The website offers this guide in Hindi, but Hindi is not currently an app-interface language.
