Keep the exact GLP-1 product in your record

A free record for the product you were prescribed and the pack you received, so a brand name or presentation change does not erase your history.

Record more than the ingredient name

If your prescription and medicine pack use different-looking names, or a future prescription changes the product, keep each record identifiable. Write the exact brand, ingredient as printed, presentation and pack details. “Semaglutide” alone does not describe every detail of a particular product. This worksheet helps you prepare a question for the pharmacist; it cannot establish whether two products can be substituted.

India’s CDSCO public drug database includes product-specific entries. The Health Ministry’s March 2026 notice also addresses unauthorised GLP-1 sales and promotion and emphasises medical supervision. Neither a search result nor a record in Capydose is a prescription, proof of authenticity or approval of a seller.

Make a separate identity line for each pack

  1. Copy the label. Record the full name and the tablet, pen, cartridge or other presentation actually supplied. Do not select a similar-looking product just to complete a field.
  2. Keep strength separate from the amount taken. Copy the strength wording as label information. Record an administration only from what actually occurred under your prescribed instructions. This sheet does not convert mg, mL or syringe markings.
  3. Keep the batch and expiry. Transcribe them carefully, keeping a private copy of the receipt. A batch number and an order reference are different identifiers.
  4. Preserve dates. Record the dispensing date and the dates of events separately. If a clinician changes your prescription, start a new identity line without rewriting the earlier history.

The download contains blank columns for label details, the source of those details and a question for the pharmacist. Use YYYY-MM-DD for dates. If you copy an expiry printed as a month and year, preserve that format instead of inventing a day. Open the CSV using UTF-8 and a comma delimiter, or print the table through your browser. No health information is entered into this webpage.

Use official lookups without overinterpreting them

When checking an alert, retain the exact product name, batch and date of the notice. The CDSCO alerts page separates NSQ and spurious-drug information and provides date filters. Failure to find a match does not certify your pack. If you cannot interpret a listing, show the label and the source page to the pharmacist instead of drawing a conclusion from partial matches.

Fictional record example: “New pack received; label wording differs from the previous pack. Ask the pharmacist to explain before recording the product.” This is not a real case or a reason to switch medicines. Keep the response and its date beside the question. For a concern about a medicine already used, seek professional advice rather than waiting to complete the spreadsheet.
